In completing your claim form ensure that the following guidelines are met.

A copy of the claim form can be found inside your policy kit. You can also download a copy from our website at www.jalpa.org - Attach all medical documentsFor Example:
• Physician’s summary and/or medical records
• Diagnostic and lab tests results
• Prescriptions
• Medical equipment request - Enclose all original receipts
For Example:
• Invoices
• Proof of payment - Send your documentation within 90 days from the date of service to:
